2017-1 - Stonegate Community Health Centre

REQUEST FOR PRE-QUALIFICATION SUBMISSIONS FROM GENERAL CONTRACTORS
Project: Stonegate CHC, Toronto.
The new Stonegate Community Health Centre (“CHC”) is to be located on the second floor of a new two storey commercial/retail building located at the corner of Berry Road and Stephen Drive in the City of Toronto.
The proposed 1,272 SM(15,000 sf) CHC will be providing space for primary health and dental care; health promotion; a teaching  kitchen; and a wide variety of community programs to the Stonegate-Queensway community (the “Project”).
The Project is funded by the Ministry of Health and Long-Term Care and will adhere to a stringent capital planning process; and will comply with, a Class “C” HVAC, the CSA Standard Z317.2-15 and Infection Prevention and Control (“IPC”) Standards. The successful proponent MUST have previous experience in new construction and/or tenant fit-out of Community Health Centres, Family Health Teams and/or Clinical/Dental space.
 
General Contractors wishing to prequalify for bidding must be union contractors and shall submit a completed CCDC Form 11 providing experience in the types of projects and requirements noted above. General Contractors are welcome to include additional supporting information demonstrating expertise in these types of projects to a maximum of 10 double-sided pages.
 
Construction start is anticipated by early October, 2017.
 
At the time the Project is tendered, the following will apply:

1. General Contractor Tenders will be accompanied by a 10% Bid Bond;
2. Agreement to provide a 50% Performance Bond and a 50% Labour and Materials Payment Bond; and,
3. The Project will be in accordance with a CCDC 2, 2008 modified.
